Most of our insurance companies have the option for you to pay your premiums by EFT (Electronic Funds Transfer), e-check or by credit card. Fast online payments can prevent lapses in coverage and save you time.

Certificate of Insurance
A document issued by an insurance company/broker that is used to verify the existence of insurance coverage under specific conditions granted to listed individuals. More specifically, the document lists the effective date of the policy, the type of insurance coverage purchased, and the types and dollar amount of applicable liability. Auto Policy ID Card Request Form
If your Auto Policy ID Card is lost or stolen, call May & Associates Agency, Inc. to request a replacement card for your current insurance policy. The Auto ID Card provides proof of coverage and also provides the insured with information on what to do in case of an accident.
